New Patient Checklist
Thank you for choosing Minimally Invasive Spine Specialists. We look forward to meeting you and helping you regain a healthier life. In anticipation of your first appointment we ask that you bring the following items with you. They will help us correctly evaluate your spinal needs. If you have any questions, be sure to call us at 708-283-2225.
- Photo ID
- Insurance Cards
- HMO Insurance Referrals from PCP if you are an HMO patient.
- Written doctor referral if you have one.
- MRI’s, CT Myelograms, CT scans, Bone scans, and X-Rays if you have them. PLEASE bring in the original copies you were given. We often cannot use discs or other digital records for diagnostic purposes due to their quality.
- Results of EMG, Bone Density Test or other test results which might be relevant.
- Please print and fill out the FIRST APPOINTMENT FORMS.
- Co-pays. We accept cash, check, Visa and MasterCard.
